Regenerative Injectables Using Your Own Platelets and Growth Factors

At The Surgeon’s Spa – Beachwood and The Surgeon’s Spa – Westlake, platelet-rich plasma (PRP), platelet-rich fibrin (PRF), and biofillers allow us to harness your body’s own regenerative potential to support smoother, healthier-looking skin. These treatments use components from your own blood—such as platelets, fibrin, and growth factors—to stimulate repair, collagen production, and rejuvenation from within.

PRP and PRF have long been used in medical fields such as orthopedics and wound care. Their ability to enhance tissue healing and regeneration has made them increasingly valuable tools in aesthetic medicine as well.

How PRP and PRF Are Prepared and Used

Your treatment begins with a simple blood draw, similar to what you might experience for routine lab work. The small blood sample is then placed into a centrifuge, which spins at high speed to separate it into different layers.

From this process, we can isolate:

  • Platelet-rich plasma (PRP)
  • Platelet-rich fibrin (PRF)
  • Other components that may be used to create biofiller

When injected into targeted areas, these platelet-rich preparations release growth factors that:

  • Encourage collagen and elastin formation
  • Support improved skin texture and elasticity
  • Help rejuvenate the skin and underlying tissues

Unlike traditional fillers, which primarily add volume directly, PRP and PRF are primarily aimed at stimulating your own tissue for gradual, natural-looking improvement.

PRP v. PRF: What’s the Difference

While both PRP and PRF come from your own blood and contain platelets and growth factors, they differ in composition and behavior:

Platelet-Rich Plasma (PRP):

  • Typically has a higher concentration of platelets.
  • Is often used in liquid form.
  • Has been widely used for indications like hair restoration, certain surgical procedures, and in combination with microneedling (for concerns such as acne scarring).

Platelet-Rich Fibrin (PRF):

  • Contains platelets plus white blood cells and fibrin, which provides a natural scaffold or matrix.
  • Releases growth factors more slowly over time due to the fibrin network, potentially prolonging its regenerative effects.
  • Is frequently used in aesthetic applications such as lower eyelid and lid-cheek junction rejuvenation, especially when paired with biofiller techniques.

The choice between PRP and PRF—or a combination—depends on your goals, the treatment area, and the specific protocol recommended by your provider.

What Is Biofiller?

“Biofiller” refers to injectable treatments that use your own blood-derived components to create a gel-like material with regenerative properties. Because the term is sometimes used loosely online, it’s important to understand how it is defined at The Surgeon’s Spa.

Under Dr. Goldman’s guidance, we use the term biofiller to describe a gel created from:

Albumin: A protein found in the blood’s plasma (often collected as platelet-poor plasma, or PPP). When gently heated under controlled conditions, albumin forms a soft, semi-solid gel.

PRF (or PRP): Mixed with the albumin gel to provide a rich source of growth factors and regenerative cells.

The resulting biofiller gel can be injected similarly to traditional fillers, but its primary goal is bio-stimulation rather than immediate large-volume augmentation. As growth factors are slowly released from the gel matrix, they can support improvements in:

  • Skin quality and texture
  • Elasticity and firmness
  • Fine, etched surface lines

Because the response builds gradually, biofiller is often performed as a series of treatments, typically spaced over weeks to months, and can be combined with other therapies like conventional fillers, topical retinoids, lasers, or surgery.

How Biofillers Differ From Traditional Fillers

Traditional fillers such as hyaluronic acid gels are designed to:

  • Add noticeable volume right away
  • Reshape and contour specific areas
  • Provide structural support under the skin

Biofillers, in contrast:

  • Tend to provide subtle, softer volume
  • Focus more on long-term tissue quality and collagen stimulation
  • May take days to weeks to reveal their full effect

For many patients, the most effective strategy is not “either/or” but thoughtfully combining traditional fillers with bio-stimulating techniques to address both structure and skin quality.

Areas Commonly Treated With PRP, PRF, and Biofillers

PRP, PRF, and biofillers can be used in a variety of ways within a comprehensive aesthetic plan. Examples include:

PRP:

  • Hair restoration protocols
  • Supportive use in selected surgical procedures
  • Combination with microneedling for concerns such as acne scarring

PRF and Biofiller:

  • Lower eyelids and the lid-cheek junction, where delicate tissue benefits from softer, regenerative approaches
  • Fine, etched lines at the skin’s surface
  • Selected areas where improving texture and elasticity is a priority

Your provider will help determine which combination of regenerative injections best matches your goals and how they may fit into a broader program including fillers, lasers, or surgery.

PRP, PRF & Biofiller FAQs

Neither approach is universally “better.” Traditional fillers are excellent for restoring shape and volume quickly, while biofillers focus more on improving the skin itself over time. Many patients benefit from a combination, depending on their concerns and anatomy.

Some patients notice early improvements in skin glow or texture, but the most meaningful changes typically develop gradually over weeks to months as collagen production increases.

Regenerative protocols often involve a series of treatments, particularly for biofillers or hair restoration. Your provider will recommend a schedule based on your starting point and goals.

Ideal candidates are typically those who want to enhance skin quality or support hair growth using their own biologic materials, either alone or in combination with other treatments. A consultation is essential to determine which option, or combination, fits best with your overall plan.
